1. General Information
These Terms and Conditions constitute a legally binding agreement between:

(hereinafter: “Provider”)
and any individual using the website dsh-matiazh.org (hereinafter: “User”).
By contacting the Provider, booking a consultation, or using the Website, the User confirms acceptance of these Terms.
2. Nature of Services – Important Legal Notice
THE SERVICES PROVIDED THROUGH THIS WEBSITE ARE NOT MEDICAL SERVICES.
The Provider offers information-based consultation services limited to:
- explanation of medical terminology,
- clarification of medical documentation,
- explanation of laboratory reports and discharge summaries,
- verbal interpretation of medical documentation during consultation.
The Provider:
- does NOT diagnose medical conditions,
- does NOT provide medical opinions,
- does NOT recommend or prescribe treatment,
- does NOT modify treatment plans,
- does NOT replace consultation with a licensed healthcare professional,
- does NOT provide telemedicine or healthcare services.
All information is educational and explanatory in nature only.
Any decision regarding health, diagnosis, or treatment must be made exclusively with a licensed physician.
3. Consultation Process
- The User contacts the Provider via email or contact form.
- Prior to consultation, the parties agree on:
- scope of documents,
- format (online or in person),
- duration,
- fee.
- Minimum consultation duration: 30 minutes.
- Written medical translation is a separate paid service unless agreed otherwise.
4. Fees and Payment
- Fees are determined individually depending on complexity and volume.
- Payment terms are agreed prior to service delivery.
- If cancellation occurs less than 24 hours before consultation, the fee may be non-refundable.
- If consultation has started, fees are non-refundable.
5. Limitation of Liability
The Provider shall not be liable for:
- medical decisions made by the User,
- interpretation or misuse of provided explanations,
- health consequences arising from independent decisions.
The User agrees not to treat the provided information as medical advice.
6. Confidentiality
All documents and information submitted by the User are treated as confidential.
Medical documentation is used exclusively for service delivery and not disclosed to third parties unless required by law.
7. Governing Law
These Terms shall be governed by the laws of [Jurisdiction – e.g., Ukraine / Poland / EU Member State].
Disputes shall first be resolved amicably. If no agreement is reached, disputes shall be resolved by the competent court.

By submitting medical documentation, the User gives explicit consent to processing health data for the purpose of consultation.
Consent may be withdrawn at any time.
4. Purpose of Processing
Data is processed for:
- providing consultation services,
- communication with the User,
- invoicing and accounting,
- legal compliance,
- website analytics (see GA4 section below).
5. Google Analytics 4 (GA4)
The Website uses Google Analytics 4 (GA4) to analyze website traffic.
GA4 may collect:
- anonymized IP-related data,
- device type,
- browser information,
- general geographic location,
- website interaction events.
Important:
- IP anonymization is enabled.
- GA4 is configured not to collect health data.
- No medical documents are processed within GA4.
- Consent banner is implemented in accordance with GDPR and ePrivacy Directive.

Users may withdraw consent at any time via cookie settings.
6. Data Retention
Data is retained:
- for the duration of service provision,

Medical documents are deleted after consultation unless legal retention is required.
7. Data Security
The Provider implements:

- password-protected devices,
- secure hosting infrastructure.
Health data is treated with heightened confidentiality and restricted access.
8. Data Recipients
